Students who borrow from the Student Loan Fund (SLF) and wish to continue borrowing in the 3rd semester of 2023 **who are studying according to the curriculum** (Borrowers who borrowed from the 1st semester /2023 only) Ask students to proceed as follows.

????Documents that must be sent

1. Loan withdrawal form for semester 3/2023 *Documents must have a clear barcode and watermark. Can be printed in color or black and white.

Use only A4 size paper*, 2 sets (Tuition fees must be filled in according to the invoice that must actually be paid. but must not exceed the amount that the Student Loan Fund lends)

2. One copy of student ID card (sign the correct copy)

3. Two copies of the borrower's ID card (sign the correct copy)

4. Copy of ID card of the legal representative *Whether there will be a legal representative or not, see from the loan disbursement form*, 2 sets.

5. Invoice for Semester 3/2023 *printed from the Registration and Evaluation website, 1 set (sign a correct copy)

(Borrow only for living expenses, not required)

6. Class registration report, semester 3/2023 *Printed from the Registration and Evaluation website, 1 set.

(Sign the correct copy) (Requires the loan only for living expenses)

7. Documents for name change/surname change *In the case of changing surname* use 2 copies of each (sign the correct copy)
